Our Space

Evergreen is a traditional long-term care facility in design; however, visitors are pleasantly surprised. They comment that the space is comfortable, home-like, smells nice, and has inviting staff. The facility and yard are secure and locked 24/7 with keypad entry/exit for all exterior doors.

  • Built in three phases:

    • Phase one (2006) rooms 1-14;

    • Phase two (2009) add rooms 15-35;

    • Phase three (2012) add Sunrise rooms 201-220 and Suites 101-107.

  • Pet friendly environment with chickens, rabbits, birds, fish and dogs of our own

  • Large central solarium with fir beams, sky lights and lush green gardens

  • Two centralized kitchens with adjacent dining rooms allow for choice and flexibility for all meals and snacks

  • Several small areas for dining, sitting, reading, conversation, privacy, listening to music, or TV watching

  • Two rooms with accessible tubs, one located in the Main building and one on Sunrise

  • All laundry is completed on site

  • Designated outdoor smoking area for residents

  • Outdoor secure walking area for residents living with dementia or other cognitive challenges

  • Balcony on Sunrise where residents can eat out, sit and soak up some sun, watch the activity on the street, and where hot dogs get barbequed

  • Outdoor patio space on the main floor with tables and chairs, flowers and plants, bird feeders, and decorations outside of resident’s windows

  • 54 single rooms with en-suite - 23 m² (247.57 sq. ft.)

  • One double room with en-suite - 46 m² (495.14 sq. ft.)

  • Seven one-bedroom self contained suites – 69 m²  (743 sq. ft.)

  • With the exception of the original rooms 1-14, all rooms have accessible showers

  • Rooms are equipped with overhead lift systems for those requiring assistance for positioning and transferring

  • All rooms can be locked and are equipped with a call bell system

  • Individual heat and light control mechanisms

  • Furniture included: electric bed, wardrobe, six-drawer dresser, bedside table, over bed table with wheels, shade and privacy blinds

  • Suggestions of what to bring: Comfortable lift chair that is easy to clean, seating for guests, small table, mini fridge, TV, Phone

  • Residents are encouraged to personalise their space with photos, artwork and other decorative touches

  • Additional charges for cable and telephone

  • WIFI is complimentary

Suites

  • Fully serviced/ Self contained

  • Small kitchen, living room

  • Washroom with accessible shower

  • Five of seven suites have an additional nook for a single bed or office space

  • Each suite has an exterior door to a small patio

  • Hospice and Respite suites come fully furnished, including an additional single bed, and hide-a-bed couch for guests

  • Suites used for long-term care/couples come with: six-drawer dresser, bedside table, over bed table with wheels, shade and privacy blinds.  There are two closets in the suite. *Bed options to be discussed.

Our Communities

Evergreen is divided into four sub communities to accommodate the wide range of ages and abilities living in Evergreen.

  • “AB” (Rooms 1-14);

  • “CDE” (Rooms 15-35);

  • “Sunrise” (Rooms 201-220);

  • “Suites” (Rooms 101-107).

Each community is unique in its vibe, flow and care team. There are times when, in the best interest of a resident’s quality of life, the resident may be moved from one community to another. This has beneficial outcomes, and is only done when the benefits outweigh the risks. For example, a resident may experience a progression of dementia, resulting in the community they live in being overstimulating, and resulting in a move to another community. Our residents’ dignity and comfort are our first priority.

AB

This is a small community with 14 rooms for people with moderate to advanced cognitive disorders. The community is essentially a straight hallway with resident rooms lined on each side, and doors on either end for access to a circular walking path. The floor plan allows for more supervision compared to a large unit with lots of nooks and crannies. People living with moderate to advanced dementia are very vulnerable and require a high level of supervision and support. As dementia progresses and independence diminishes, protecting the residents’ dignity, privacy and comfort are the top priority. This small enclosed space supports this.

Some residents from this community dine in the main dining room and take part in group activities located throughout the facility. Others spend most of their time in this community. There is a small dining room and kitchenette in the community, as well as a small secure patio.

CDE

The CDE community is made up of 21 single rooms, and one double room for couples. All rooms have private three-piece bathrooms. The community is separated into three short hallways surrounding the nursing station, all with access to the main dining room, central kitchen, solarium and TV rooms. The community is ground level with exits to the secure outdoor space.

This community is suited to those with complex medical conditions, or those with full physical dependence as a result of stroke, advanced dementia, or frailty. During the day, each Health Care Assist (HCA) has an assigned group of four to five residents. Residents wake when they choose and the HCA’s assist with personal care, medication administration, nutrition, walking and so much more.

Sunrise

The Sunrise community is an independent space located on the second floor of the facility. It has its own kitchen and dining room with 20 foot ceilings, large windows, and sliding door to the outdoor deck. All 20 rooms are single rooms with a three-piece en-suite. The twenty rooms form a square surrounding the central living room and common areas. There is a comfortable bathing room with a large accessible soaker tub.

This community is well suited to respite and transitional care, as well as long-term care residents that are interested in more opportunities for social interactions and engagements. The Sunrise community has more of a social vibe than our other communities, and most residents are in a mild to moderate stage of illness.

Suites

The one-bedroom self contained suites are fully serviced, including 24/7 care teams. The suites are licensed for and are ideal for long-term care, hospice palliative care, respite care, and are a great option for couples. They are located on the ground floor of the Sunrise community. Those living in the suites join the Main or Sunrise communities for meals, or recreational and engagement opportunities.

The suites are spacious and accommodating for families. Within each suite is a kitchen, living room, washer/dryer, two closets, and all but two suites have a separate small nook for a single bed, office space, or craft room. There is a separate entrance for family members and other visitors, in addition there is reserved covered parking for guests of Hospice.
