
Medication Management
The care team is knowledgeable and experienced in the pharmacological management of many chronic diseases and symptom management and will support you in managing your medications to the greatest extent you prefer. Generally, the care team administers the medications but if you would prefer to manage yourself, a self-administration care plan can be put in place.
The medications are reviewed and reordered every 6 months, usually through a process where the pharmacist, physician and nursing team review current vitals, applicable lab monitoring and other areas and provide suggestions and recommendations as needed.
The nurses will manage all ordering for routine and as needed medications; the resident or representative will manage the pharmacy account. All medications are packaged and delivered from Campbell River Health and Drug to Evergreen.

Advanced care planning
The multi disciplinary team will work closely with you to develop your advanced care plan.
Advance Care Planning is the process a capable adult follows to identify their goals, values, and beliefs related to their health and care. Advanced care planning requires open and honest communication about the illness and trajectory and quality of life goals. Areas we may discuss are, nutritional goals, activity goals, use of medications, use of higher levels of care, appropriate monitoring and diagnostics, safety such as falls management and access to psychosocial and spiritual support services. Together we plan how to treat issues that arise and proactively address issues that we can anticipate.

Bathing and Hygiene
The Evergreen team will assist with personal care to the extent you would like. Minimal or full assistance with getting dressed, eating, washroom, washing, bathing and grooming.
Most resident washrooms have an accessible shower and there is a large accessible soaker tub room located in each building. Residents are encouraged to shower and bath as often as they would like, you are not limited to a bathing schedule. Some people require assistance for part or all of the bath, others only require assistance getting in and out but would like to soak alone.

Engagement and Recreation
Recreational Services is responsible for the promotion and delivery of care that enhances the quality of life of each Resident. Programs are designated to meet the social, therapeutic, recreational and rehabilitation needs of all Residents in the Home.
An activity calendar is posted in each room as well as on the information bulletin board in the common area. There are a variety of group events, entertainment, games, exercises, movies, music, news, outings and more planned for activities. The calendar will also include as many opportunities outside Evergreen as possible such as movies, shopping, picnics, trips to restaurants, to the pier, museum and other local places. We strongly encourage Residents to attend events of interest to them. It is our goal to assist Residents to remain as mentally alert and physically active as possible.
Activities are offered in large groups, small groups and on a one-to-one basis depending on the type of activity and the needs of each individual person.

Hair Salon & Foot Care
Enjoy pampering by our qualified hair dresser Sarah Grant. On-site full service hair and nail care available on Tuesday and Thursday.
There are two registered foot care nurses working at Evergreen. The cost for foot care is $XX and completed every 4-6 weeks or as otherwise discussed. Foot care services are provided following obtaining consent for the service. The costs will come from the comfort fund set up in trust.

Housekeeping & Laundry Services
Evergreen provides a weekly cleaning of the resident room as well as weekly laundering of bed linens.
Evergreen provides laundering of personal clothes. Sometimes families continue to do the laundry. All clothing requires labeling, even if the family choses to wash. Towels, linens and blankets are included in the stay with laundering completed on site.

Dental and Vision
Evergreen uses a mobile dental hygienist service, consent for this service is obtained on admission. The cost for the services is withdrawn from the comfort fund held in trust. Other residents continue to see their dentist and/ or denturist. Evergreen can assist with transportation and companions for an additional charge. The care staff will assist with oral hygiene, cleaning dentures and cleaning glasses.
Residents are encouraged to continue with vision checks. Evergreen can assist with transportation and companions for an additional charge.
Residents are encouraged to have their glasses labeled.
