Long-term Care
Long-term care provides 24-hour professional care to individuals and couples over the age of 19, in a supportive and secure environment. Our inter-disciplinary team works hard to meet the complex health needs of those in our care. The average length of stay is 12-24 months.
Assistance is provided for all aspects of care, including disease, symptom, and medication management, personal care and hygiene, transferring, mobilizing and repositioning, nutrition, and laundry and housekeeping. We offer a robust engagement/recreation program to provide social, spiritual, physical, and psychosocial support.
The multi-disciplinary team consists of the resident (s) and family, physicians, nurses, care aides, psychology, pharmacy, physiotherapy, kinesiology, occupational therapy, dietary services, recreational and engagement therapy, and hospice services. Care planning is a continuous process within our team with the commitment to promoting, and maintaining maximum comfort and quality of life.

Services:
24-hour professional care and care planning with a multidisciplinary team
Food, fluids, snacks in collaboration with an onsite Nutrition Services Manager
Routine laundry service for bed linens, towels, washcloths and all articles of clothing that can be washed without special attention to the laundering process
Routine weekly housekeeping services, as well as daily room care by all care team members
Recreation and Engagement program consisting of a variety of physical, social, purposeful, spiritual and recreational activities, experiences, opportunities. This includes religious services, music, dancing, bingo, concerts, movies, outings
Specialized foot care nurses, dental hygiene, physiotherapy, occupational therapy, and hair salon services are available at an extra charge
Costs
In long-term care, there is a provincial mandate for the hours of care that are required for each person in care, which directly dictates funding. That provincial amount is 3.36 hours per person in care. At Evergreen, we staff for more than 4 hours per person in care – well above the provincial amount required.
The daily rate for Evergreen’s long-term care beds is $320.93, whether publicly or privately funded.
Publicly funded long-term care beds are a shared cost between the resident and the Ministry of Health. Private pay services are the responsibility of the individual or a third-party payer, if involved, such as Veteran’s Affairs Canada, Indigenous and Northern Affairs Canada, or Extended Health Care such as Blue Cross, Green Shield, etc.

Admission
To be assessed for private care, the first step is to complete the application for accommodation. Upon receipt of this application, we will confirm receipt of application via email, and then review eligibility.
